APPLICATION INSTRUCTIONS
- Select all of the Companies you would like to get appointed with.
- Download, Save and Print each document.
- Fill-in all paper work with a "Black Pen"; (please print and write legibly).
- Fax or mail us a "copy" of your current State(s) License that you wish to be appointed in.
- Fax or mail us a "copy" of your Errors and Omissions Insurance (E & O). If you do not have E & O Insurance, please let us know and we can assist you with this. A copy of your E & O insurance must be presented to us within 90 days of working with our Agency. The majority of the companies we work with require it.
- You may fax all of the completed documents to:

PartnerTeq is the company we have known and trusted for over 8 years. PartnerTeq has built all of our sites and hosted them as well. We have been in close contact with them recently, trying to see if we can put together a packaged deal that will help our agents get started.
